The recommendation is a 3 year re-test for ELF in the BMJ article also, so that will need costing in. Also, some consideration would need to be given as a gatekeeper test in primary care. The use of ALT as part of the standard liver function test profile, is a very imperfect surrogate of the degree of necroinflammation and fibrosis in NAFLD. We've just been approached. I have said I'd be prepared to help draw up a business case as the test will cost us £50 a time.
The clinician responded that he's fed up with asking his managers to get money for tests as it doesn't seem to achieve anything!
