Used to work on agency nurse pay at the Royal College of Nursing (rare example of a 'trade union' (sort of) unilaterally setting pay rates that were widely followed!).
Basically this involved transposing NHS pay rates for nursing grades to the agency context. So assuming people get paid much the same as NHS would be appropriate.
The Agency/Bank thing is that agencies and banks actually do some work recruiting, contacting and managing the temp shifts. On top, agencies have a margin (of course). It is possible that the Bank services have been cut back in the various cutbacks of administrative staff in the NHS, and also that their management of the temp service may be more or less efficient than the private sector.
One major issue is that some temp staffing in nursing (used to be a lot) is done by NHS employed nurses in their spare time. There have been attempts to clamp down on that due to tiredness resulting in poorer care etc. Banks likely to take such requirements more seriously than agencies.
Not very surprising that agencies seeing a demand, and with public sector competition forbidden or limited, might see a profit opportunity. Though I think NHS admin does have limits on such charges.

Your posts ideintifying the same source as I found were sent while I was still in the process of doing my own search, and then composing my own posting about it. I did not download and read emails recived during this period until after I had posted, so we had all been running the same course but I kept running with my eyes shut after you two crossed the finishing line before I got there!
Well, at least independent searches came up with the same result, so evidence is beginning to accumulate that this may be the unique source of the information.
John's comments about "pigeon-holed" funds is interesting.
