Dear Clare
(Nice reply James!)
Here's an example: a group of GPs were running an EBP Journal Club in Mackay in North Queensland (in the steamy tropics). They looked for evidence about whether after skin excisions -- very common for skin cancers up there -- the wound has to be dressed and kept dry as per the guidelines. This is a serious impost for patients in the tropics -- no showering, no swimming... A search found no good evidence. So they simply set up a trial, and at very low cost run a well-designed study that showed -- no you don't! This is it:-
Heal C, Buettner P, Raasch B, Browning S, Graham D, Bidgood R, et al. Can sutures get wet? Prospective randomised controlled trial of wound management in general practice. BMJ. 2006;332(7549):1053-6.

One thought is that when one knows the evidence around an area, one then knows what we don’t know as well. I would suggest knowing what we don’t know really opens up creativity.
