It might well be worth searching Visitation records. For instance those of the visitations of bishop John Attwater of Lincoln between 1517-31 contain references to many disturbances in church during service time:
Visitations in the Diocese of Lincoln 1517-31, Edited by A. Hamilton Thompson, Lincoln Record Society, 1940, 1944, 1947. On page 92 of Vol I, for instance, we find reference to "Willelmus Aukland et Willelmus Holme ludebant as taceras [dice] Sabbato Palmarum tempore divinorum".

More on parish churches than cathedrals ... but you will find the literature on the demon Tutivillius a good starting point.

I'd be grateful for more references on this myself. I always felt that one of the weaknesses of that otherwise excellent Experience of Worship project was that we didn't spend enough time thinking about the congregational experience and particularly about what congregations were told not to do (and presumably therefore were spending a lot of time doing). That was really why I behaved so badly in the Saturday Mass - no-one else was doing it so I had to be naughty for everyone!
