Suggest measure triglycerides and compare with TPN.
Also google chyle composition for several references.
with best wishes

Subject: Chyle composition
Dear all
I would be grateful if you have any suggestions regarding this unsual query.
The lab was contacted by paed surgical team regarding a neonate who is on both aqueous and lipid TPN. The baby was investigated for the possibility of perforated gut and on exploration his gut was healthy but milky fluid was found in his abdomen and reteroperitoneal. The team wanted to investigate whether this fluid is chyle or leak from PN.
As the patient is nil by mouth so it is unlikely to be chyle. Is there a test to confirm that it is PN? we have looked at trace element composition of blood, PN, and fluid for comparion and results were as follow
Fluid [Se] 0.12umol/l
Blood [Se] 0.32 umol/l
TPN [Se] 12.4nmol/kg
The fluid is hyperosmolar with an osmolality of 1570 mosmol and that means that this fluid contains electrolytes, glucose.... Does anyone know the actual composition of chyle? Any suggestions?
Many thanks
Soha
