In the Engager II project (http://clahrc-peninsula.nihr.ac.uk/project/55-engager.php ) we've used realist review (and additional data sources) to specify how a complex mental health intervention (for offenders) is proposed to work. It includes both organisational and individual-level issues, such as the relationship between practitioners and offenders, and offenders’ perceptions, understanding and skills (and how these interact with available resources).



The clinical interventions you mention (and the environments in which they’re implemented) may differ in important ways from the mental health intervention and environment that is our concern in the Engager II project – but the methods for articulating the complexity of how interventions are proposed to work may well be judiciously transferable. I have a general question for the list. Most realist evaluation and review to date has focused on ‘programmes’ such as change in health service organisations or to implement changes in practices, etc. I’m interested to know how many people have used it to address more clinical questions to do with the ways in which specific interventions work. Clinical interventions are often thought to be less complex, but in maternity care at least, most a complex and become more so the more you look into them.



We are planning a realist review of a clinical intervention because we feel conventional systematic review methods don’t allow sufficiently for complexity and will be interested to hear about others’ experiences of doing this, including whether you have adapted your approaches at all.

Following numerous requests and Gill's kind but unwarranted approbation I have quickly assembled a resource to support Literature Searching for Realist Reviews:

It includes my own CARES presentation and all the other essential searching methods items in my personal "Desert Island" Collection. I am hoping that Chris Cooper will also make his CARES presentation available to us.



So if you have emailed me for search guidance over the last few days (thanks Gill :-) )please look at this resource first. Of course I will then be happy to answer specific questions arising from the resource materials (and of course any resource I subsequently find useful in answering your follow up questions).



As in true realist fashion I have privileged relevance and timeliness over rigour I will be very interested in learning of anything you find useful that is not yet included on my resource :-)
