Dear Luke,
Just a quick note to express that I strongly oppose the use of the expression "design problems" in such terms.
In my view, "design" can solve problems, but there is no such thing as a "design problem", specially for the people who actually feel the detrimental effects of said problem.
I just went through surgery. I had a small hernia. What I had was not a "medical problem" or a "clinical problem", but a "health problem". For the surgeon it was surely a "medical problem," for some of the staff it was an "administrative problem," for some it was a "financial problem," a "technical problem," etc.
I know I'm rattling the cage of the semantical beast, but not getting this kind of thing straight tends to muddle discussions. And it gets you on the wrong mindset, for you should be thinking about the people and not about the design.
