We have worked in co-design often, as example, for designing science games
for young people and as well for a communication systems with cancer
patients (http://proyectocpr.wordpress.com). I have literature but is in
Spanish.
Answering two of your questions:
“… what are the best practices of co-design?”
Empathy, it has helps us be aware of the others context, at the same time,
we are aware is not necessarily ours so we don’t know their experiences.
“..finally are there specific methods utilizing co-design that you all have
utilized and found success in what you were trying to accomplish?”
We use rapid ethnography (we don’t have much time to design and developed),
follow up by written guides to the designers, so they have a protocol to
use in the activities; and reflective practices, so we check on the process
our ideologies, expectations and motivations. Also we trained the staff to
be comfortable with public errors and wanderings.
