Thanks to everyone who responded on & off list, and to the COPAC Helpdesk team - we've now got this issue resolved. The problem was caused by a combination of command line mapping (of 998 fields) in our z39.50 load table and some changes in the RLUK database.
Shirley Cousins at COPAC explained: "In the past we had to generate MARC exchange records on export so we didn't incorporate local content, whilst in the new database the MARC exchange data pre-exists so it now includes a range of local fields. " And Shirley added "our apologies for the work that this has created".
Of course, it only caused work for those of us with long-forgotten mappings of 9xx fields in our load tables ;-)

p.s. this is separate from the Sierra problem related to downloading records with diacritics from any database. I'm afraid that still exists.

We're having a weird problem when downloading records via z39.50/remote search from RLUK. Basically when we try to download certain records, it causes the Sierra client to crash. We've managed to establish that the problem affects records from a small number of contributors to RLUK including Univ of Southampton; Institute of Education & Queen Marys, both Univ of London; Royal Academy of Music. No problems with any Millennium/Sierra libraries! And no problems with z39.50 downloads from other databases e.g. LC, OCLC.
The problem started about 2 weeks ago, coinciding with official launch of new RLUK database but also when we upgraded to latest version of Sierra, so not sure which event might have caused this.
Is anyone else experiencing this or anything similar?
