Fiona, I think your below conclusion is correct.
There is a CLSI C49-A guideline on "Analysis of body fluids in clin chem" which gives a very good overview on the topic.
The below papers are also quite helpful in interpreting results for the dg of chylothorax:
- Chest 1999;116;682-687
- Paediatric Respiratory Reviews 10 (2009) 199–207
These papers, amongst additional information on interpreting lab tests in pleural fluid, propose a TG cut-off for chyle of 1.1. mmol/L.

Thank you all for your advice. The consensus seems to be that running fluid triglycerides and cholesterol on whatever platform you have available is warranted but with a note of caution to the requestor about assay validation.
In this particular patient fluid triglycerides = 0.2 and cholesterol = 0.54 with no visible chylomicrons after centrifugation therefore unlikely to be chylothorax.
