If you're getting a blank page I strongly suspect you should also be
getting an error message in the PHP log files; if not, ensure that PHP
error logging is turned on. Most likely there is a syntax error in one
of the PHP files. Sorry, I do not have access to an LDAP server to test
this at my end.
If you're looking to integrate with Moodle then I would definitely
recommend that you look at using LTI; the connector can be found at:
http://www.spvsoftwareproducts.com/php/webpa-lti/
and if you're interested in a solution which allows the synchronising of
enrolments and groups then check the guide available from:

From what I can read from the errors I have established connection to our LDAP (im not presented with a connection error), I get a "Your username and password were rejected. Please check your details and try again.", when I tried logging in with a LDAP user. If I use the local db user, the login is successful.
