Looking at "File --> Properties" when this PDF is opened in Acrobat
Reader shows "Author: Dr Neil Bhatia". This may provide a lead.
Also visible is:
"Created: 19/11/13 22:27:59"
"Modified: 10/01/14 19:121:24"
Googling on "Dr Neil Bhatia" locates him at the Oaklands Practice,
Yately [Hampshire] Medical Centre: http://www.ymcentre.info/staff1.aspx
He seems to be strongly involved in information protectionn etc.,

However, note that the four opt-out codes 9Ndo/9Nu0/9Nu4/93C1 which
are discussed by Dr Neil Bhatia are not the same as the opt-out codes
given in the "Care.Data Patient Choice Form":
XaZ89 for option 1 (not "to be shared outside GP practice")
XaaVL for option 2 (not "to be used by other healthcare organisations")
[quoting the wording on the form from my GP practice]
so, despite the apparent clarity of Dr Neil Bhatia's document, I still
wonder to what extent it is relevant to the current Care.Data issue!
