A haematologist colleague asked my opinion this morning on the following;
"Background:
A recent article in Blood suggested that for patients with paraprotein and renal impairment the condition should be called “Monoclonal Gammopthay of Renal Significance – MGRS”. However since both paraproteins and mild renal impairment are both common what assessment should haematologists make in this situation?
Question ;
In patients who present with a small paraprotein and mild stable renal impairment who are otherwise asymptomatic should haematologists undertake an assessment of renal protein excretion?
If yes what test should be used and what actions limits should be used to trigger e referral to a nephrologist?
