Dear all,
I have received a request from one of our endocrinologists for a patient with possible reactive hypoglycaemia to have an 5 hour glucose tolerance test to include insulin measurement.
I have never come across this but what little I can find in pub med seems to indicate that it is probably not very useful.
Does anyone have any experience of using this test and a protocol that we can use or evidence that we shouldn't be using it?
Reading through the clinic letter I think it seems unlikely that this patient has reactive hypoglycaemia as she seems to be needing to eat every 1-2 hours to avoid "feeling hypoglycaemic" although an 8 hour fasting glucose was normal. I would have thought the more appropriate test would be a 72 hour fast? We are also awaiting results on 24 hour urine mets.
