We use a locally created Filemaker database which runs off a server version of the program.  It allows us to use either a client or a web interface.
Having a locally developed system allows us to intergrate instruction into the same file and to be able to have "specialty" fields. One specialty field is if the question required a reference librarian - so was the question difficult enough that a support staff could not have reasonably answered it.

If we were not using this - I would probably recommend we use RefAnalytics in LibAnswers from the LibGuides folks at Springshare.  It seems sufficiently flexible that it would meet most of our needs.  We use LibAnswers so it would be easy to record email and chat transactions.
Since when we are on the ref desk we have LibAnswers open for chat and email questions, it would make sense to record the other ref stats there.
Finally- the cost seemed pretty reasonable to me though I guess that depends on your funding.

There is an open source program LibStats - http://code.google.com/p/libstats/ but it is not being developed.

We looked at Desktracker - http://www.desktracker.com/ - and it looked reasonably good - but we thought the pricing was excessive.  (again - local
perspective)
