Depends which field of design you are looking at. I'm guessing you are
restricting your picture to the Art and Design fields.
Other fields of design that have more obvious literatures that connect
design activity to political outcomes are:
Electoral system design;
Design of Governments
Political deliberation systems design
Systems design (many fields)
Political engineering
Policy system design
Development program design
Policy design
Legislation design
Taxation system design
Design of Social Change
National Innovation Systems Design
Community development design
Social program design
Design of socio-political change
Socio-economic program design
Engineer and Society literature
Professional engineering literatures - many different fields
Association of Computing Machinery (ACM) libraries
Red Rock Eater and Phil Agre's other writings
As well, of course, the large design-related literatures in the realm of STS
(Science and Technology Studies)
In each of these are examples of designs of interventions that resulted in
rapid change.
One stage back are the design research theories (many from AI design)
underpinning the design of computerised systems enabling designers to
design much faster and produce designs that are more reliable in their
outcomes - which in turn has enabled better and faster design of radical
interventions in social systems.

currently reading Tony Fry's book on "Design as Politics", but am looking
fro more concrete / practical examples on how design has achieved to change
