Currently for both of these types of request we ask for the following:
Report Title
Place/Date of publication
Report number
Cited in
Other info
Which I guess relates to Q2, Q3a and Q3b of Innovative default prompts - plus some of the prompts which appear for all.
As Geraldine has mentioned below it would be also nice if a Series field could be included.

We use the following fields for Government and Technical reports- but we've only done a handful of these requests in the last few years so they aren't a big concern.

I had a question from one site about Government Reports and Technical Reports and how these will be handled when the changes are made to ARTEmail. I had not mentioned these two request formats up till now as we don't actually use them in Durham (we instead mis-use them as a way of allowing users to submit serviced photocopying requests). We just make users fill in the book request form, or another form, for this kind of document.
The British Library do not give any recommendations as to the field
order for these types of request. Innovative's default set of prompts
only has
Q1: Sponsoring agency
Q2: Title
Q3a: When/where published
Q3b: Report number
If anyone is using these request types to send requests to the British Library, could you please send me a list of the fields you use for the bibliographic information in these requests as soon as possible.
