we did similar a couple of years ago following all steps as outlined by Jonathan Kay.
Everyone was happy except for one GP commissioner, who felt we hadn't informed them of a change of practice with sufficient notice and were therefore in breach of contract. It was all smoothed over in the end but worth remembering its not just clinicians we have to deal with these days.
