Zbys, Douglas and Pranab, when I replied to Zbys about the spelling of bollocks I should have prefaced my remarks with *irony alert*.
Neal et al, I don't really care what term or what acronym people use (EBM, EBP, SBM, ...). What I do care about is that healthcare providers use the best available evidence to provide the best possible treatment, guidelines, and policies. The qualifiers "best available" and "best possible" indicate that (i) due diligence has been done, and (ii) available resources are limited (and the limits will vary geographically).
Debating about the supposed differences between EBM and SBM is useful only to clarify the concepts that are essential to articulating what the aims of EBM are.

I tend to agree, Michael - this idea that EBM = RCTs seems to have
spread like a virus. It's interesting how people have latched onto the
Levels of Evidence to demonstrate EBM's positivist fascism.
However, I think that the root of the issue is, as is often the case,
with the Research Question.
EBMers, by and large, are considering the question of "What is the best
thing to do for my patient or population?", whereas SBMers seem, on the
face of it, to be dealing with the question "What is knowledge?"
Very different concerns!
