Exiftool ( http://www.sno.phy.queensu.ca/~phil/exiftool/ ) will do this, here's an example command line I've used recently.
exiftool -if "not $Title" -T -r -FileName -Title . > out.csv
This tests to see if anything is in the Title metadata field and if not outputs the Filename and Title (as a double check), it will also recurse subdirectories from whatever directory you run it from. It creates a tab delimited text file called out.csv which you can them load up in a spreadsheet.
I just included the conditional test here to show how powerful exiftool can be, and is very handy for QA testing. If you wanted to just dump various metadata fields you could use:
exiftool -T -r -FileName -Title -Headline -XResolution -YResolution -ISO . > out.csv
Any XMP field that exiftool can see you can output, I use ExifTool GUI ( http://freeweb.siol.net/hrastni3/ ) to see what field names ("Tag Names") are in an image along with their content.
