We can barely do it to 1!
Recent UK NEQAS data shows that results on an innocuous 2.4
mmol/L calcium the histogram spans wider than 2.2 to 2.6 on a 38g/L
albumin. see 855C. The Total and Adjusted Calcium data are very
similar.

Pathology Harmony has recommended deriving an equation for
calcium
adjustment for specific combinations of Calcium and Albumin method.
All users of the same platform type(and chemistries) should in
principle be able to use the same adjustment equation. It's proposed
that the equations are normalized to a serum calcium of 2.4 mmol/L
and
that an adjusted calcium range of 2.2 - 2.6 may then be universally
applied (summary attached)
As to the lower limit of albumin, adjustments are usually adequately
accurate down to at least 20 g/L, but any deviation from the linear
regression at the lower end should help to inform.

Hello,
Could any of the list members tell me if they have an Albumin lower
limit restriction when calculating Corrected Calcium, using the
formula:
Ca(corrected, mmol/L) = Ca( measured, mmol/L) + {(40 - (Albumin) x
0.02}
We use the BCP method for measuring albumin and an Arsenazo-III
dye
for Calcium.
I would be interested in your comments as to how accurate you
consider
this formula to be at Albumin levels less than 25g/L and at what level
an ionized Calcium would be more appropriate.
