I am very puzzled by this discussion but share Yvonne's point about the result being recent.
I do not remember in the original documents anything about a 'shelf life' or expiry date for results.
Frequently, agency staff request a new result yearly - I would question the rationale behind this but obviously we would do the test to ensure that this member of staff can continue to work via that agency but I believe to be unnecessary.

Hi Ann,
Why does their serology need to be recent? If they had had their primary course and a decent level why make them retest to provide a recent serology; the levels will have dropped and then you will need to boost & re-test surely?
We ask for a level of > 100 and evidence of 5 year booster only (we do not have EPP staff)

When a health worker who will be EPP status comes into our Trust, they have to provide a validated serology report that is no older than a year for their Hep B status either/or antibody and antigen. If they have been in EPP work prior to 2002, that is all they need to provide. If they started EPP work post 2002 but before 2006, they have to also provide evidence of their Hep C status, and after 2006 they have to include HIV.
Any one who has had a full course and their booster need not be re-tested routinely providing that they had a good immune response at the time of the initial course. That is quite often where staff do not know, so we test them, find their level is low, then we start the - do we give them a booster, another course etc, etc.
I agree that the information on line is vague, so it will depend upon your local policy as well as the current information. We do not routinely re-test after the 5 year booster.

it still doesn't address the issue though....
what I am trying to find is some documentation which advises and/or states when doctors have to provide proof of their Hep B status (if such a thing exists). If it doesn't then my problem is solved.
I'm not looking at routine course, blood test, booster, blood test but whether routine checking after those have taken place is advised ie after 5 years post booster....
any thoughts most welcome

It is all EPP workers, rather than just doctors, that require specific tests such as validated blood samples to prove immune status.

Hi Vicky
the DH guidelines below don't address routine testing after vaccination and booster; it is this point which I need to get some clarity on...any ideas?

is there any document (DoH, NICE etc) with guidelines as to what is required....as always, it seems different hospitals require different things
thanks

what I am referring to is:
The management

for NHS staff
New edition, 2005
There is currently no routine requirement for the testing of healthcare workers other
than doctors for blood-borne viruses. However, if they think they have been exposed,
healthcare workers have a professional duty to seek medical advice on the need to be
tested for blood-borne viruses.
which states doctors have to have routine testing for BBV so what is routine???

Hi Chloe,
for EPP workers, we require evidence within 2 years (UK lab, Identified validated sample), of Hep B surface antigen, as well as evidence of completed course & antibody response (from whatever date).
For non-EPP, evidence of course of vaccine, response & booster (or date required)

Hi Chloe,
No.
Once you have had a full course you either have no immunity start again, partial immunity with immediate booster and a one off five year booster or full immunity with a one off five year booster.
Immunity determined by titre level. In NHS provided we see lab report/result from other Occ Health then that is it.
After these dates we only test if no result available and we need to update file.
Booster offered after a needlestick injury.
Hope this helps
Clive

Dear list
Please can those involved in Hep B vaccination advise with regard to their policy on checking Hep B status: after booster, do you require further proof of Hep B status and if so, how often.... both EPP staff and non EPP
many thanks
