I am concerned at some of the nomenclature in this questionnaire.
The old problem of 'tests' and 'requests' arises with no guidance for requestors or laboratories. This is compounded by asking elsewhere for 'number of samples' - yet another variable.
I am also concerned at the perception of an electronic request.
We are currently receiving a plethora of request formats designed by clever people in the GP community which I call IEDs (improvised electronic devices). These cause problems due to poor design and layout frequently resulting in missed requests. My concern is that the requesting GP might well regard this as "electronic requesting" and fill in the questionnaire accordingly.

As electronic ordering and reporting systems are being to be more widely used concerns have been raised about the quality of administrative data in requests and reports.
The NPSA in partnership with the professional has commissioned a survey into current experiences in order to obtain baseline information on current practice.
