I wonder how the original question was posed? There is a body of work
on asking people, including doctors and the general public, to make
difficult choices. Most doctors I know are aware that, at some point,
there is a clash between the rights of the patient in front of you and
the rights of the others, still to visit. That no health service has
inexhaustable resources.
They can also outline the general advantages and disadvantages of
using QALYs, or some such economic approach, to resolve these
dilemmas. I certainly hope my students can do so...
Maybe I meet the wrong doctors.

It is not strange that physicians do not like this analysis as to decide about to pay or not for a treatment is a policy decision, not a single physician decision. Physicians should pay attention only to effectiveness and report moral objection to the use of cost. How ever politician should consider costs since money do not serve only the purpose for health care problems but for other expensive social services too.
