(I work in a children's 'department' so there may be some differences.)
1. We don't sign children on with their own logins etc. we have a generic
password for each of the three computers and use that to sign on. The details
we take are for parental permission (name, address, library card no., and
permission) which are filed and not used as login data.
I believe that upstairs on the adult PCs they just take a proof of ID the first
time and then set up an account.
2. We don't monitor screens ourselves, but we have pretty strict filters (a
little too strict in my opinion) which keep people away from almost anything
with pictures.
