We use the item holds here and, in fact, make a big point in training (which I carry out) that staff are not to put bib holds on. We have had a couple of incidents when this was done accidentally, and we needed to contact Innovative to have them removed.
We also have a policy where items need to be returned within one week when they are recalled, so which copy is selected doesn't really matter in most cases. The loan period will be reduced to one week automatically.

We're reviewing our lending policies and its been suggested that we move to using bib-based rather than item-based holds. This would mean that the reserver gets the copy returned soonest rather than whichever copy they've clicked on in the OPAC. However concerns have been raised that this might cause confusion and delays.
As a quick straw poll, I'd be interested in knowing who in the UK uses which type of holds. Especially useful would be if anybody has switched from one type to the other, and if so why?
