I think the poem is called "Farewell, old Playmate" by Egmont RUSCHKE.
This poem is included within a book called "The Poets of the Future: A College Anthology" by Henry THOMAS, published 1918.

Open it using the 'flip book' option - I tried using PDF and HTML, etc but they didn't work on my PC! :-(
The poem is on page 130.
