The test done by the Police when they suspect you of driving
with excess alcohol is a one-tailed test. You would NOT want
them to adopt a two-tailed test.
Of course, that is a sample of 1. In the domain of environmental
monitoring (when a company, say, is required to keep emissions
below prescribed levels) you would see a one-tailed test based
on a larger sample size.
Coming closer to Medicine, surely there must be circumstances where
health authorities are montioring disease levels for evidence of
exceeding a threshold at which a public intervention would be
called for?
