I agree with Bill. Dynamed, InfoPOEMS and Cochrane are good places to start if you have a subscription. The TRIP database is a more comprehensive source of multiple types of sites including internationally published guidelines and it also searches PubMed using their "Clinical Queries" feature. I try to get my students to use TRIP in searching for evidence at the point of care. The biggest challenge is to get the students to go beyond Up-to-Date rather than appraised or original sources of evidence.

Generally, I recommend Dynamed, Infopoems, Cochrane, or TRIP - usually in that order - Pub Med as a last resort for rare or obscure questions, or when the others are not helpful.
