Im meant -r (not -R): You may also try -F to trick it? Given that you have Fat Sat turned on, it might be worth a trial. If both don't work, you may well send an image to me to have a look. Send it to me directly so it does not spoil the list.

Andreas,
I applied a number of bet variations to the original and intensity-inverted
scans, including -R and specifying my own center of gravity. None worked. I
may be able to send you a nifti volume if you'd like to work with it yourself.
Unfortunately changing fat sat isn't an option, as the clinicians have
decided to use this sequence as-is.
