Two documentaries: _Dances Sacred and Profane_ by Dan & Mark Jury, with
Charles Gatewood as "tour guide", produced in 1985, shortly after the
Re/Search _Modern Primitives_ book rolled out the more extreme end of
these practices to "general hipsterdom"; and the 2005 _Modify_ by Jason
Gary and Greg Jacobson. The latter is easily available on DVD (i.e.,
Amazon) the former from this site: http://www.dancesfilm.com/
_Dances_ is about other things too (it's sort of a revisioning of the
"Mondo movie"), as it's basically covering the breadth of Gatewood's
work, so e.g. there's a long section on Mardi Gras, stuff with Annie
Sprinkle, etc., that may be of varying interest for your class, mixed in
with the tattoo convention and the segments with Fakir Musafar.
It feels almost too obvious to mention, but _Videodrome_ and _Crash_
(the Ballard/Cronenberg, not that other thing) both address your (1) and
(3).
For limits of perception, well, I'm always happy for another chance to
push Corman's _X - The Man with the X-Ray Eyes_. Tony Conrad's _The
Flicker_ is worth the 16mm rental if you can get your classroom dark
enough for it to work. Tons of experimental films, actually, depending
on how you wanted to spin it ... you could do this entire course out of
the Canyon Cinema catalog and skip the features if you wanted to ;^>.
If you wanted to add a literature component, I'd highly recommend Kathe
Koja's _Skin_, which touches on most of your themes ... but it might
give your campus bookstore a hard time as it's been out of print for a
while (check ABE, Amazon, Alibris, etc.).
