Have tested this again, and Anna & Leo are correct: the problem has
gone away. It was at least 18 months ago we were experiencing this - it
helped scupper our early moves towards an open holdshelf. We've been
through a couple of software releases since then, so maybe it was fixed
along the line somewhere. Just goes to show I need to keep re-testing
these issues ...
On a related point, do people perceive any data protection issues with
having patron names on held books on an open holdshelf? (I notice Anna
refers to putting just initials on the holdslips) We keep our holdshelf
in classmark order, and quite often have several copies of the same text
sitting there...

Hi Richard,
I have tested this and it only does it if it is still out to patron A
and patron C tries to borrow it. If it has been returned by patron A and
is on the hold shelf for patron B, patron C is blocked from borrowing
it. So it should work correctly, unless it is not returned and users
collaborate with each other (not very likely).

Yes, it does cause problems, but in a different way (or at least it
did
last time I tested it). If the book is on holdshelf for patron A, and
patron B is further down the queue, a self-check machine will allow
patron B to borrow it.
Cheers,
Richard

Further to Leo's enquiry, does the existence of a subsequent hold (i.e.
a hold queue) on the item being self-checked from an open access
holdshelf cause any problems for (i.e. prevent) that item from being
self-issued? (I ask because I know the existence of subsequent bookings
on an item presents a problem for self-issue of those items)
