Hi Richard,
I have tested this and it only does it if it is still out to patron A
and patron C tries to borrow it. If it has been returned by patron A and
is on the hold shelf for patron B, patron C is blocked from borrowing
it. So it should work correctly, unless it is not returned and users
collaborate with each other (not very likely).

Yes, it does cause problems, but in a different way (or at least it
did
last time I tested it). If the book is on holdshelf for patron A, and
patron B is further down the queue, a self-check machine will allow
patron B to borrow it.
