We have self-check machines and we haven't seen this problem at our library. Patron B would not be able to borrow the book on the holdshelf for patron A. Patrons do sometimes try to borrow the wrong book because we only put their initials on the holdslip but the machines say no if the book isn't on the holdshelf for them.

Yes, it does cause problems, but in a different way (or at least it did
last time I tested it). If the book is on holdshelf for patron A, and
patron B is further down the queue, a self-check machine will allow
patron B to borrow it.
