i know not from Carthusians, but my understanding is that the 17th c. practice
among Reformed Cistercians (Trappists) was that the novice monk dug his own
grave, early on. somewhere i've seen a woodcut --a kind of cul-de-lamp-- of
this rather dramatic scene.
whether or not this was the custom amongst middlevil Cistercians i do not
know.

i haven't a clue.
for the Cistercians, Chrysogonus Waddell at Gethsemani is a Font of Knowledge
--including about Things Trappist-- and is Generous to a Fault with it.
