The problem really seems to be conflicts of interest (which is actually a
polite term for something else) affecting academic medicine. Most of the
manipulation of clinical research and of EBM itself could not take place
without the complicity of faculty and their institutions who are far too
often financially entangled with organizations with vested interests in
promoting their products or services. The entanglement is complex, and
often goes to the top of the academic hierarchy.

Until we end (not just disclose) these conflicts, starting from the top, we
will not address this problem.
