u should simply send a medical report plus charge (if appropriate), having
satisifed that it kosher to give the info, and state that unable to attend.
recently i had a lady who was being lifted by her rels to be cared for as
the formal carers from ss etc are not allowed to lift for h+s reasons. aids
in home useless. now rels cannot lift as pt needs frequent care and turning.
ergo, pt now has to go into NH against her and her rels wishes and at
considerable expense simply cos the ss 'carers' wont lift.
