Our cystic fibrosis unit have been using i-STAT for a few months and are very pleased with them. They are based in one of the peripheral hospitals in the Trust (with no lab on site) rather than the community, but the principle is similar.
The service was implemented after consultation with me, I provided wthem with published evaluations and went up a couple of weeks ago to see how they are doing and to check procedures. The analyser is held by the Senior Physio and she allows only trained staff (register kept) to use it. They do QC and EQA and keep proper records.
I think it's essential that any monitoring in the community is implemented only with lab advice and that the need for training, quality and proper record-keeping is accepted. One problem I can see would be refrigeration for the cartridges - perhaps some sort of cool-box?

Dear colleagues,
Our Respiratory CNS's are looking to purchase several portable blood gas analysers that are suitable to transport to the homes of patients who are unable to attend the hospital for monitoring their oxygen requirements.This is in line with the Domiciliary Oxygen Therapy Services Clinical Guidelines published by the Royal College of Physicians. Does anyone have experience of acquisition and use of such analysers? Advice gratefully received,
