I´m not sure what you mean by "break your heart"...
Are you looking for examples where (literal) metaphors
are presented in a film as if they "real" (bear with
me, just my own non-post-Lacanian description).
In case you do, you might want to try this:
Manfred Riepe: Bildgeschwüre. Körper und Fremdkörper
im Kino David Cronenbergs. (Applying Lacan / the use
of metaphor to David Cronenberg´s films, I don´t know
whether there´s an English edition).
Other uses of "headache-scenes: Lost Highway ("Aspirin
- my head" // Fred Madison´s transfiguration scene)
Pi - the movie (the protgaonist visualizes his
headache which is a common method in therapy I
believe, but with his brain as something external.
