Thanks to all of you who responded for requests of copies of forms. I
did receive a few copies which has been really helpful, but as Tim
Padfield at TNA rightly pointed out, I cannot post the various lists I
received to the list. However, if anyone is interested, I can let them
have my form which is really an adaption of a few of the forms I
received. Tim also pointed me towards TNA's policy on the subject:

The main considerations to take into account were:-
Supervision of readers whilst the photography was carried out.
Mobile phones cameras were not allowed.
Flashes and tripods were not allowed.
Some archive want the person who will receive the copy to sign the
form rather than the one doing it on their behalf.
The Archives retain copyright and permission must be sought if
publishing or passing on to someone else for their own research.
Usual copyright declarations
Particularly fragile documents cannot be photographed.
No scanning.
