i believe that the custom was a universal.
certainly the oldest depictions of the chapel of Notre-Dame du Pilier at
Chartres show ex voto objects, and they can be found on pilgrims' badges and
stained glass windows from the 13th century onwards.
it seems that the early modern and modern "housekeeping" swept most of them
away (or sanitized them into placques, etc.) in the more "advanced" parts of
Europe.
