Dear Rob
How would we know that 80% of designs fail? Surely 100% of designs fail or, as Bob Dylan says:
"There aint no success like failure and failure aint no success at all."
The famous stone bridge across the river Wey is made of wood.
The famous golden buddha was always known as being made of bronze - it was a buddhist joke - turns out it was made of gold - it was a double joke.

All written material is to an extent fiction and fiction can convey
misconceptions as well as usefull ideas. This may explain also why 80%
of new designs fail. Failure is a necessary part of success and
innovation.

It is sometimes forgotten that myth serves a positive purpose. i.e.
myths arise for very good reason. Primarily it is a way of coping with
complexity that is beyond (and may remain beyond) our capacity to
analyse, describe and understand. Humans have limitations, our own lack
of requisite complexity is one of them - we are part of a world which,
because it contains us and millions of actors like us, is more complex
than we are. Simplification is necessary but changing.
