Kriss-Rettenbeck illustrates quite a few Byzantine silver ex voto
tablets. That might fit for the Tolentino shrine. I'm not aware that
they were common in the west.

A number of wax body-part ex votos was found tucked away in
Exeter Cathedral early in the 20th century. See U.M. Radford, “The
Wax Images Found in Exeter Cathedral,” Antiquaries Journal, XXIX
(1949), 164-68, pl. XIX-XXI. As a general phenomenon, your leads in
the history of medicine are right on; there is a good section on them in
Bénézet, Jean-Pierre, Pharmacie et médicament en
Méditerranée occidentale (XIIIe-XVIe siècles) (Paris: Honoré
Champion, 1999).
