Have heard of similar case with interesting cause...
Pt was 17, female, obese, in status ep, all investigations normal.
GP clinical assistant wandered into resus, examined abdo and asked if
anyone had done pregnancy test.
...diagnosis concealed pregnancy and eclampsia.
Once seen (or heard of), never forgotten.
