Googlise "Chad Perry marketing thesis" and look for his 5 chapter thesis model. There's one version at http://www.tele.sunyit.edu/Thesis-Approach.pdf - better are the older ones from the literati club and Southern Cross. Software - Endnote is pretty well a defacto standard. Endnote 6 has some useful extras - the travelling library is a useful add on for people writing papers with multiple authors.
