I'm not a medical librarian nor in the NHS, so maybe I don't understand the
context properly.
However, in higher education we have similar issues and its not about
eligibility for Athens or the costs of creating Athens accounts that is
important, but what the licences for electronic resources say about who can
use them. Most just talk about members of your organisation, though some
give clearer definitions. In HE generally access to electronic resources is
given only to staff and students - even external library subscribers do not
get access.
Do the Core Content licences define users? Do they specifically allow
walk-in use?
