Who's auspice's do these NHS family members fall under? Is it the regional health authority? the local PCT? The regional workforce development confederation (presumeably them seeing as it's they who fund the core content). Whoever that is it would be their librarian who'd issue passwords wouldn't it?
Would it be of value for the regional confederations to issue librarians with a list of "affiliated organisations". Perhaps this information could be posted on NELH or local confederation websites. I for one wouldn't have a clue what organisations in Birmingham are eligible - apart from the obvious ones.
If there are no cost implications to the number of Athens passwords issued then fine, it doesn't really matter does it. If there are cost implications then we need further clarification about who qualifies and who doesn't.

It would certainly be neater for us if it was as black-and-white as this,
but the press release announcing the Core Content specifically says "The
collections offered by Dialog and ProQuest will be available to the entire
NHS family including staff working in hospices, health related charities and
health and social care staff." And Scott is saying something which implies
an even broader definition.
Your second point is the one I am drawing attention to- the Scott-defined
group is so ragged round the edges that we can legitimately do as you say.

surely "full membership" should also include the setting up of an Athens
password. imho it's simple, not employed by the nhs - no password.
If "Full memebership" only covers borrowing and I'll's then we as librarians
can issue Athens passwords willy nilly can't we?

I asked Scott Gibbens how elastic the concept of 'the NHS family' is, for
the purposes of using the NHS Core Content, specifically for a BUPA nurse
undertaking a course at a college of higher ed, who was using us on a
reference basis, possibly just once or possibly as a regular visitor. Here
is his answer:-
Anyone can access the core content resources on a walk in users basis, but
that would be dependant on you allowing access to your library and the nurse
being able to come in to use the resources.
Otherwise, unless the BUPA hospital is contracted to provide ongoing
services to the NHS, she cannot have access. She should however be able to
get access to electronic resources via her Higher Education establishment.
Given that we allow anyone (including the general public) walk-in reference
use, but not full membership (defined as book-borrowing and free
inter-library loan rights), this leaves a bit of a barn door open. Views
anyone?
