The FSA Hospital is considering the feasibility of combining all the patient
records that are now kept in the individual wards (departments) and placing
them in a single collective archive. At present every ward keeps its own
patient records (archive). Consequently an individual patient may have
records in all the wards of the hospital.
Could you please inform me as to the arrangement of these matters in your
institution? If you keep a collective archive for all the wards, we would
appreciate details on the following:
1. How do you keep your patient records? (different wards in the same
archive?)
2. The storage and delivery of patient records. Who is in charge?
3. Are the records bar coded and charged out like for instance books in a
library?
4. Do all the individual wards have access to a patient’s records in all the
other wards - unconditionally?
5. Are all the records perhaps electronically registered so that there is no
need for the storage of paper documents? If so, what kind of system has been
set up?
