You could try looking at the AOI building block Events Manager - it
looks like it does the trick to me, but I have to persuade people higher
up than me to pay out for it.
(that's an enterprise option though)

Subject: Help sought in creating a sign up sheet
Dear list members,
I've received the following query from a colleague about how to create
an online sign up sheet and wondered if any of you could make good
suggestions? The course in question has about 100 students.
Many thanks,

Query:
"Any ideas how I would go about creating a simple sign up sheet via the
VLE [we're using Blackboard - zoe], where students can sign up
electronically for e.g. appointment slots.
All I need to set up is a list of available times and have space for
them to be able to add their names next to a given time electronically
to save them having to come into the Centre and sign on sheets outside
the office?
I never really explored the quiz feature and wondered if it could be
done through that perhaps or some other way?
Perhaps there is a dinky bit of software floating around somehare that I
could use - I know I could set something up in Access or similar for
example, but would that work?"
