We too have some errors with labels on the tubes not matching labels on
the forms. We have a double check process at reception and then after
computer data entry/before instrument loading to ensure that results are
ascribed to the correct patient.
In addition we used to receive on request per month from A&E where both
labels on form and sample are the same but belong to the wrong patient.
We now have Pathology phlebotomists in A&E for 12 hrs per day.
However it is no worse than it was in those dim distant days pre
labels.
Now at least we can read the labels and use the bar codes - the clarity
of writing on label on a curved tube is so variable.
The efficiency of the phlebotomists is also substantially higher, just
over 75% more patients bled per hour in outpatients per phlebotomist.
